How do I apply?

A two-step process determines eligibility for HARTPlus services:

​

  • Completion of an application that is sent in advance

  • An in-person interview and functional evaluation, if ncessary, to determine individual circumstances. (Transportation assistance is provided to the interview if necessary.)

​

Information provided by a social service agency or other professional regarding the applicant's ability to use the bus may also be considered. This process takes as little as 21 days.
